As a transportation tech, I visit a lot of rehab facilities, and North Mountain stands out. The staff here are always friendly, organized, and ready to help—not just with patients but with coordinating smooth transitions in and out. The patients always mention how good the food is, and the atmosphere is warm and welcoming. One of the best-run facilities I’ve seen.

My dad received care here for 3 days and actually thank goodness they messed up bad enough that he had to go back to the ER and get out of there. We were very hopeful after Jeff gave a compelling tour that it would be a great stopping place between a hospital discharge and going home as we learned how to care for my dad. It was as bad as the reviews (and I hate to post this as I know how desperate families are to find a place). At the hospital he was walking with a walker easily but upon arrival he was no longer allowed to walk until he was "cleared" by the PT more than 12 hours later (he wasn't allowed to go the restroom, or even allowed to step on a scale, instead they used a sling to lift him when he was clearly ambulatory). The first night they clogged his J tube which he needed for feeding, they put food in his G tube which is not permitted. Then they clogged his G tube so for some period he could have no food or medicine. They spilled hot coffee and Coke on him while trying to unclog the tube, offered him a PB&J (he was nothing by mouth), and seemed to make up medical procedures as they went, despite family members who were well versed in his care being there and advising. A doctor refused to speak with my dad or brother when discussing the J-tube care. It is wild how long the list is in the short time he was in their care (there is more!). Ultimately their negligence with the J-tube required an ER visit that allowed him to get out of there after 3 nights and we would never send him back. In fact, when my brother went back to pick up his supplies from the room they were gone and it wasn't until several days later the items magically reappeared. Unorganized, unprofessional, and nothing like Jeff sold it. Beware.

My son was there for just over a year. I was not impressed with the care he received or the facility. The employees were not happy or friendly. My son often sat in his soiled or wet brief for longer than he should have. His food tray would be left of his table and he had to wait 20 to 30 minutes for someone to come feed him, so his food was cold. I tried calling the administrator and the Director of Nursing several times, I only was able to leave a message and never had my call returned. He only got bed baths, when I inquired why I was told he refused. When I talked to my son, he said he did decline because the shower chair was broke for the entire year he was there and the shower bed was much to short for his 6'9" frame. It made his legs hurt. The only person I received any help from was the patient liason, she puts out the fires in that place. I gave them 2 stars because my son was able to be transferred closer to home. I would not recommend this facility for a loved one.

A relative spent eight days as a patient at North Mountain Medical and Rehabilitation Center, a place I wouldn't recommend for anyone. The patient achieved his discharge goals two days after he arrived, but he was held for another six without any reason or, in fact, discussion, with the administrators even though we asked to meet repeatedly. The facility is understaffed, so call buttons are usually answered only 30-90 minutes later, if at all. All phones go through a main line, which isn't always answered even during business hours. A social services person finally met with us and told the patient that he could always leave AMA, so he wasn’t a prisoner there (insurance doesn’t pay for someone who leaves AMA). "Prisoner" was her word; we didn’t say it. She said that the center lets patients "acclimate" to the center before the case manager and other administrators meet with patients to review plans for them. She said the meeting occurred two weeks after arrival; written material left with the patient days after his arrival said three weeks. While the physical and occupational therapists were good, the last therapist the patient worked with, the day before the patient was discharged, told him how surprised the therapist had been to see his name on the patient list, assuming that he had been released days earlier. The center kept the patient for an additional day after his discharge papers came through. The center released the patient without his prescriptions, including a new one. I called the center three times about this, including telling them that he was out of the medication. They wanted me to go back there to pick up the prescriptions, which they knew wasn’t an option. Three different times that I spoke with nursing staff or administrators, they told me they would mail them. They didn’t. They sent me pdfs of the prescriptions, which pharmacies legally can’t accept. After hearing from the center again asking me for a survey and review, which I promised I would provide to every review site I could find, the "case manager" contacted me. That was the only time the patient or I spoke with her. On the pharmacist’s recommendation, I told her to have the doctor's office phone in the prescription to the pharmacy; she said their doctors don’t do that. I told her to mail them; she said their mail doesn’t work well. She finally brought the prescriptions out to our residence, but by that time the patient had gone two days without his new prescription.
