I had previously gone to a different therapy place and was not pleased with what they were doing as they didn’t seem to be concentrating on what the problem really was. I went there for three sessions a week for a month and never felt that they had done anything for me. I found Spooner therapy through my insurance company and have only been going there for 2 weeks now and I’m amazed at the difference in how I feel since I have been treated at Spooner. I would highly recommend this place if you need physical therapy!

I am extremely excited to start my Physical Therapy. My Physical therapist is absolutely amazing !!! Cassidy is out going and super easy to talk to. Shes a great listener and understands the issues I’m having. I explained the issues I was having per her response “we will work on ALL of it”which was awesome to hear. I had a great experience at Haven Health Sky Harbor. The facility was always clean and well-maintained, which really gave me peace of mind. Staff is fantastic, Too many to list but a couple stand outs: Samantha, Justin, Sarah, Olga, the list goes on. The facilities are nice and well kept. They go above and beyond my expectations and I am grateful for it. Even if there is a problem they work to resolve it efficiently and quickly. The food is good, portions are reasonable. Alternatives are equally as good. I have been to many other facilities this by and far the best. Far as Staff, far as accommodations, if there is a better place I haven’t found it.

The respiratory and therapy teams deliver care with a passion for their field, this isn’t just a job for them and it shows in the superb care that they give!!
Case management offers discharge support even in the toughest of discharges, always doing their very best to ensure the safest possible discharge. They are easy to work with and quick to respond to an email or returning a phone call.

I would send my family member here without a showdown of a doubt !!

They specialize in this kind of treatments. It’s a 24 hour care that’s above and beyond. Staff has tremendous amount of patience and are the most qualified and skillful to care for the residents. The difference in their operations is noticeable and incomparable, which is built around what is best for the residents , something that is lacking almost everywhere. Came here for IV antibiotic treatment for 2 weeks. I must say having been to one of these facilities before it was the worst, but not this place! The staff was amazing anything I needed they were on top of. Very professional but also caring at the same time… To be honest I really didn’t want to leave lol. Shout out to the entire staff, give them all raises. Thanks for getting me back healthy.

The worst care ever. Avoid at all cost. They leave the patients in their soiled clothing or diapers for an hour or two before they actually tend to the patient. That’s after the call nurse button has been activated, the the CNA would come in and deactivate the call and leave without helping the patient. Calls to the front desk go straight to voice mail and administration and Social Worker never return phone calls to answer needed questions. I can’t get any help for information from anyone there! Calling the Arizona Department of Healthcare about this place! No one should be subjected to such poor health care. My friend has endured horrible lack of care at this facility.

My family member was at Camelback for the most part the staff was nice. The facility was not. Multiple times i had to clean up my own family member after they had been sitting in urine and stool so long it caused an infection that’s resistant to antibiotics. Their response was just let them sleep every time. This place was a major set back. They don’t interact or push to motivate patients at all.

This place is wonderful so far the best rehabilitation center I been at, they really know how to work with you on the healing process they are so calm and very gentle with you, I would like to give a very special shout out to my caregivers Cassie, Ricardo, Maria.S and my nurse Develyn, Cassie is so loving and caring she made sure I was doing okay and she made my stay so wonderful she deserves 10 stars in my book, also for Ricardo he made my stay feel the same way Cassie did he is a great guy and very funny also if I could give 10 stars to Ricardo I would, Maria.S made sure we were comfortable she worked with you on keeping you busy, she would give you crayons to color with or word search that was one of my favorite things to do and kept me busy if I could I give her 10 stars also I loved her smile and cheer fullness and my nurse Develyn is such a awesome nurse absolutely very nice she would always check on you to make sure your feeling okay and puts your needs first, if I could give her 10 stars I would if it wasn’t for these guys I wouldn’t know where I would be, but I’m so happy I got to meet these guys but not just them deserves ten stars I think the whole staff members deserved 10 stars but this place was absolutely amazing and I would recommend this rehab to everyone I enjoyed my time here. I have been at this facility for almost two months and have had exceptional care from some cna’s that I am proud to mention Stacy, Aliyah, and Ricardo L. Then there has been some nurses that have had my back but when I was at my worst Tessie had me and helped me and I got the problem taken care of and felt better afterwards. I know I can count on my cna’s to take care of me and they go out of there way to come by to make sure to speak to me. STACY, RICARDO L come by and Aliyah does my hair so I look my best which makes me feel so good. I am happy to have these people taking care of me and look forward to seeing them each day.

Excellent staff, both medical and support. They go out of their way to make sure everyone is take care of, and needs and wants are met. Unfortunately this has been a bad year for me, have spent more time in hospitals and skilled nursing facilities than at my apartment, this time when asked where I wanted to go for in house phyical therapy I said “back to Haven on 20th Ave”. My lucky stars that they had room for me. Food is pretty good too!!

I was in rehab here for shingles mengitis. It was ok. Most of the nurses were attentive. Food was good. The activity directors had a array of activities planned for the residents. A barber comes and offers haircuts for cheap.
The negatives were long wait times for calls during some night and weekend shifts. Only one bathroom shared for 2 rooms which meant 4 residents had to share the one bathroom. The shower was elsewhere in the facility.
Overall Haven Health Phoenix is ran well. I would recommend it, it is better than a lot of skilled nursing facilities and nursing homes.

This place is a joke. I drove from Tucson after speaking to them no less than 10 times before coming. I waited 2 hours in the waiting room, finally went back for my assessment. A half hour later, they told me they wouldn’t take me due to a past behavioral health diagnosis. I begged them, and they sent me away. Even was vomiting in their lobby and was going through DT’s. So I ended up driving home back to Tucson — vomiting the whole way home. You’d think a behavioral health rehab would be able to handle MENTAL HEALTH. called every single place in Tucson, and no one will take my insurance. I’m now on my way to the ER, because Calvary was my last chance.

Artificial Idiot

This facility, like many treatment centers in Arizona, operates primarily within a for-profit model—something prospective clients should be aware of from the start. If your condition isn’t considered “severe enough” or if insurance coverage appears uncertain, there’s a chance you may be turned away. Unfortunately, this has become a common trend in the industry.

Compared to higher-end centers like Cottonwood or Sierra Tucson, which often resemble wellness retreats, this facility takes a more structured and stern approach. While that may appeal to some, it may feel rigid or impersonal to others. Staff members appear committed but may not always have the training, cohesion, or resources needed to provide truly individualized care.

Some of the group sessions are informative, and there is valuable content to be gained. However, the overall experience can feel transactional. Once insurance coverage ends, unless you actively seek and secure a scholarship or financial assistance, the transition out of the program can be abrupt—and for many, destabilizing. That post-treatment drop-off is a vulnerable time, and without the right support, it can lead to relapse.

Recovery is deeply personal, and it’s important to explore what works best for you. While 12-step models are widely used, they aren’t the only path to healing. Be cautious of assuming that any one center has all the answers. Treatment can be a useful starting point, but true recovery comes from your ongoing commitment, support system, and self-discovery beyond the walls of any facility.

In short: approach with awareness, advocate for yourself, and remember—your recovery is your own. Choose a path that truly supports your long-term growth and not just what insurance will pay for.

As a transportation tech, I visit a lot of rehab facilities, and North Mountain stands out. The staff here are always friendly, organized, and ready to help—not just with patients but with coordinating smooth transitions in and out. The patients always mention how good the food is, and the atmosphere is warm and welcoming. One of the best-run facilities I’ve seen.

My dad received care here for 3 days and actually thank goodness they messed up bad enough that he had to go back to the ER and get out of there. We were very hopeful after Jeff gave a compelling tour that it would be a great stopping place between a hospital discharge and going home as we learned how to care for my dad. It was as bad as the reviews (and I hate to post this as I know how desperate families are to find a place).

At the hospital he was walking with a walker easily but upon arrival he was no longer allowed to walk until he was “cleared” by the PT more than 12 hours later (he wasn’t allowed to go the restroom, or even allowed to step on a scale, instead they used a sling to lift him when he was clearly ambulatory). The first night they clogged his J tube which he needed for feeding, they put food in his G tube which is not permitted. Then they clogged his G tube so for some period he could have no food or medicine. They spilled hot coffee and Coke on him while trying to unclog the tube, offered him a PB&J (he was nothing by mouth), and seemed to make up medical procedures as they went, despite family members who were well versed in his care being there and advising. A doctor refused to speak with my dad or brother when discussing the J-tube care. It is wild how long the list is in the short time he was in their care (there is more!).

Ultimately their negligence with the J-tube required an ER visit that allowed him to get out of there after 3 nights and we would never send him back. In fact, when my brother went back to pick up his supplies from the room they were gone and it wasn’t until several days later the items magically reappeared. Unorganized, unprofessional, and nothing like Jeff sold it. Beware.

My son was there for just over a year. I was not impressed with the care he received or the facility. The employees were not happy or friendly. My son often sat in his soiled or wet brief for longer than he should have. His food tray would be left of his table and he had to wait 20 to 30 minutes for someone to come feed him, so his food was cold. I tried calling the administrator and the Director of Nursing several times, I only was able to leave a message and never had my call returned. He only got bed baths, when I inquired why I was told he refused. When I talked to my son, he said he did decline because the shower chair was broke for the entire year he was there and the shower bed was much to short for his 6’9″ frame. It made his legs hurt. The only person I received any help from was the patient liason, she puts out the fires in that place. I gave them 2 stars because my son was able to be transferred closer to home. I would not recommend this facility for a loved one.

A relative spent eight days as a patient at North Mountain Medical and Rehabilitation Center, a place I wouldn’t recommend for anyone. The patient achieved his discharge goals two days after he arrived, but he was held for another six without any reason or, in fact, discussion, with the administrators even though we asked to meet repeatedly. The facility is understaffed, so call buttons are usually answered only 30-90 minutes later, if at all. All phones go through a main line, which isn’t always answered even during business hours. A social services person finally met with us and told the patient that he could always leave AMA, so he wasn’t a prisoner there (insurance doesn’t pay for someone who leaves AMA). “Prisoner” was her word; we didn’t say it. She said that the center lets patients “acclimate” to the center before the case manager and other administrators meet with patients to review plans for them. She said the meeting occurred two weeks after arrival; written material left with the patient days after his arrival said three weeks. While the physical and occupational therapists were good, the last therapist the patient worked with, the day before the patient was discharged, told him how surprised the therapist had been to see his name on the patient list, assuming that he had been released days earlier. The center kept the patient for an additional day after his discharge papers came through. The center released the patient without his prescriptions, including a new one. I called the center three times about this, including telling them that he was out of the medication. They wanted me to go back there to pick up the prescriptions, which they knew wasn’t an option. Three different times that I spoke with nursing staff or administrators, they told me they would mail them. They didn’t. They sent me pdfs of the prescriptions, which pharmacies legally can’t accept. After hearing from the center again asking me for a survey and review, which I promised I would provide to every review site I could find, the “case manager” contacted me. That was the only time the patient or I spoke with her. On the pharmacist’s recommendation, I told her to have the doctor’s office phone in the prescription to the pharmacy; she said their doctors don’t do that. I told her to mail them; she said their mail doesn’t work well. She finally brought the prescriptions out to our residence, but by that time the patient had gone two days without his new prescription.

Anybody can put paint on a wall… or keep a room clean. What impressed me most wasn’t just the cleanliness or the well maintained equipment; it’s the Nurses, therapists, techs, doctors, and management — every single member i interacted with was knowledgeable, encouraging, and incredibly kind.

The therapy rooms are well equipped and staffed by therapists who are as creative as they are compassionate.

I had checked into a SNF
previously before checking into Reunion. At first I was nervous about my stay here because the previous Place was an extremely unpleasant experience however I quickly realized there was a night and day difference between places, that my family saw as well.
The most notable of differences here is that Reunion pays attention to the minor but important details

I had 90 minutes of physical therapy, speech therapy and/or occupational therapy per day. Zero complaints–just praise. I would recommend Reunion to anyone.

Awful! Waited 4 hours in the waiting room with my daughter for results of a covid,flu,strep tests Note there was only 2 others in the lobby and thosetypesof test take only 10 minutes. When I got up to tell them just give me the results over the window and ill just go somewhere else they took her in within less then 5 mins. The doctors didn’t seem to know what even look for, acted like they did something then told me her test all came back negative. The doctor gave her nasal spray when its her throat thats hurting. She left feeling worse then what we came in. That is awful experience.

My grandpa broke his hip and needed rehabilitation so we found Reunion and are so happy we did! The facility is beautiful, impeccably clean, with comfortable, private rooms, and has an amazing space for PT/OT. The staff was phenomenal. Jacor gave us a warm welcome and made sure my grandpa was settled in, even though we arrived in the middle of shift change. Adeline was our nurse for a few days and she had such an amazing bedside manner and kind heart. She anticipated his needs even when I wasn’t there to advocate for him and kept us both informed. It allowed me to take time for myself and get things organized for his return home. The CNAs answered call lights and bed alarms quickly and warmly. Tory, Rod and Jacob were dedicated to his PT/OT therapy and helped his mobility and ability to care for himself improve significantly in the short time we were there. My grandpa loved Lindsay, his speech therapist. He has dementia and she worked with him on some memory tools and was thoughtful enough to help him with his hearing aids so he could fully participate in his care. The kitchen staff went above and beyond to provide choices and set up his tray for him every day. I also appreciated the lovely gals at the front desk for providing us a schedule every day and being so personable, and the involvement of pharmacy staff and our case manager, Marcy. This place is top-notch, and provides the type of care that anyone would wish for for themselves or their loved ones.

It was kind of a wild experience at first as an alcohol user and definitely opened my eyes seeing the struggle of drug addiction firsthand. The staff was great and taught me a lot. The rooms were very comfortable. If any of the staff that was there in January of 2021 is still there I want them to know that I have greatly improved my life and have held a full time corporate job for almost 4 years. What a let down. After hearing wonderful things about this place by my friend Tracy who has relapsed repeatedly after successfully completing their program multiple times, I wonder if she was paid to tell people about Vogue and encourage their enrollment. I say this because it was not wonderful and it felt like everyone that was there as a patient was really an employee faking being in need of treatment but originally applied to work there as a therapist or a staff member. It felt like the twilight zone and totally fake. Nothing like comparable recovery centers who live up to their name that I give 5 stars to compared to the one star at Vogue. The place smelt so bad, it’s been months since I went there but I still get flash backs of that smell and it makes me cringe. It took them 5+ hours just to invited me back to start the check-in process. They said they’d run my clothes through the dryer for their bed bug policy but they certainly did not do that so I doubt they ever do that with any of the clients there which also makes me cringe…eww! Despite them holding all of my clothes locked up without me having access to anything until halfway through the next day so I had to wear their giant size scrubs with no bra no panties allowed until I received my items and this is a coed facility with several meetings during that block of time and I have DD boobs which made me feel very uncomfortable and inappropriate. With the men and women . Staff was very unprofessional from all areas I think because half of their staff was pretending to be patients it was probably confusing how to act or behave with one another for the staff being friends or peers colleagues with patients secretly. I think I was the only real patient there the entire time. Oh another huge complaint I was served rotten chocolate milk and it was so rotten and disgusting that it was full of large curdled like cottage cheese large curds of rotten milk that I drank and then spit up once I realized what they had served me. Just eww! Once I was approved to leave earlier than planned despite going through withdrawal which was the reason I was there in recovery for to receive help with the withdrawal they made me drag all my luggage in a big plastic bin by myself the entire lengths of the facility sweating and in tears so achy with that nasty rotten taste in my mouth and once I reached the front of the building the entrance where they took 5+ hours to even start Che king me in days before, the front desk staff told me I had to wait for my ride outside off their property boundaries and could not use their phone to call anyone for help despite releasing me. My phone sat in lockup for days so when I got it back it was only charged to 4% battery life and I had a long walk off the boundary of their property where I was forced to go, before I could even start calling for help or booking an uber. Least to say it was miserable, made my heart ache going through that experience it still does just thinking of how all that went down. I will never ask Tracy for any kind of referral again! This is my only negative review I’ve ever made and I’ve done tons of reviews on various platforms. Ugh. Woo Sahhh! Shake it off**

I had to spend 2 weeks here after a major back surgery. So glad I did. The staff is amazing from housekeeping to charge nurse. Everyone was so caring. There goal help me to get better. P.t, O.t very consistent making sure I could physically take care of myself, my nurses and Cna’s where so caring, they made sure I had everything I needed. Friendly people even the bistro and reception.

Physical Therapy Can Be Terribly and Unbearably Scary. But Northpark is FAR from an episode of South Park…with top notch managers and therapists who hit it straight out of the park!

Can’t say enough about Marla Pino-Ramirez, Isiah, and the rest of the crew at this excellent facility. Our family is truly appreciative for their professionalism and especially their patience for my elderly mother who had to undergo surgery for spinal stenosis.

Marla stressed to us that the physical therapists at Northpark would get my mom back on her feet again in three weeks. Seemed like a steep hill to climb for an 80+ year old after a major procedure, but Marla’s knowledge and confidence immediately sold us. No used car salesmen tactics. Just a major emphasis on getting my mom strong as quickly as possible, so she can get her life back.

My mother is doing well. She is back on her feet with the assistance of a walker and well on her way to running circles around us again.

So BIG shout out to Marla, Isiah and the rest of the gang!

From the moment you walk through the doors you know this is going to be a positive experience and it didn’t disappoint. The staff is friendly, caring, and attentive. They were a true blessing for him by the manner the treated him and shared conversations when time allowed. My husband never waited more than a few minutes when he pressed the button. The room is private and kept very clean. He complained about his mattress one day and they immediately replaced it. Hats off to the kitchen staff. The food was so wonderful, that I shared his mid-day meals with him for only $6. The days the served something he didn’t like, they offered alternatives. The chef checked in daily to be sure he was happy with his meals. You’re greeted by Kathy at the front check in desk, who is never without a cheery smile and welcoming attitude. The PT staff was engaging and motivating. So much so that my husband has far exceeded all expectations and being released weeks early. I can’t say enough good things about this facility. You always hope you don’t need it, but if I do I wouldn’t hesitate in the least to be their guest.

As a patient in Northpark for rehabilitation after discharge from hospital with a very painful injury. The quality of care from nursing staff and physical therapists was excellent. The food was good and all areas were very clean. My pain was managed to my satisfaction. The person (Mary Ann) in charge of coordinating my discharge was wonderful in keeping me updated. Even coming in late night to reassure me that everything was taken care of to her satisfaction. If ever you or loved one is in need of rehabilitation, I completely recommend NorthPark.

My father-in-law was here for about 3weeks. I am from Illinois and I was very impressed compared to some I’ve seen back home. The facility is very clean and nice.!! The staff, therapists, nurses, aides, D.O.N. and DIRECTOR were all very knowledgeable, helpful, and extremely nice. I would highly recommend it to anyone who needs assistance in their recovery.

My mom was placed here after her chemo treatments nearly killed her. She was in the CPOD and I will forever be thankful to this staff. The nurses were amazing and patient with her through her whole stay. She arrived there defeated and tired and ready to check out. The wonderful staff worked with her and me to give her the ability to keep fighting and she was able to walk out of there after being wheeled in. We will forever be grateful to this facility. They are communicative, patient, kind, and the facility site itself is clean and welcoming. Definitely would recommend this place to anyone who needs it.
