I saw Chelsea for my post operation for a total knee replacement. On my 1st visit she measured and gave my knee a good once over! Sent me home with PT exercises to do daily 3x a day. I saw her twice a week for 12 weeks. She had PT technicians assisting her Katie, Lilly, Brandon and Austin while I did my workout. I was able to work at my own pace too.

Scheduling was easy liked how I could “check in online ” they would charge my card when I got there.

Location was great right there at 32nd Street and Greenway.

Professional and very helpful front desk staff. The PT therapist Peter was very attentive and knowledgeable. He answered all my questions. I feel a bit better after my first session and deep tissue massage I received from Peter. Looking forward to my next session.

Austin worked with me for 8 weeks on my hip. He is very knowledgeable and used many different options to improve my strength and alleviate hip pain. I came away with less pain and many exercise options to maintain my fitness level. A side note: Austin also has a great sense of humor and good taste in music. A shoutout to Lilly, too, who worked with me.

Chelsea is consistently professional, thorough in her assessments, and attentive to both my physical need and verbal feedback. Austin is a very good communicator, very knowledgeable in his profession, very patient and a very good listener. Very pleased with this physical therapist. The exercises that Austin has chosen for my husband are excellent. They will definitely help to strengthen his upper back, shoulders and arms.

This is my second time getting PT treatment from Dr. Rachel Slack. Rachel’s calm, patient and positive demeanor helps to motivate me to show up for each appointment. She’s incredibly knowledgeable and is with me every step of the way while I get stronger to heal my recent knee injury.

Highly recommend this practice – in fact, we now have about two dozen family and friends going there too! My husband and I both see Dr. Kessler who is so knowledgeable – and personable! She’s helped us both solve derm issues we’ve struggled with for years! Also worth noting the admin side is a dream as well – professional, timely, competent. Love love love.

Dr. Baltazar was extremely caring and attentive. He took the time to listen to my concerns and provided multiple treatment options. Together, we decided on the one that suited me best. He was transparent about potential side effects, which I truly appreciated. Communication through his assistant was excellent, they consistently kept me updated on my insurance coverage and next steps. I never felt rushed during my appointment, and I left feeling very happy with the care I received. I am happy I found a provider that truly cares. A heavy piece of furniture fell on my toe while I was wearing flip flops, and it completely damaged my toenail. The pain was awful and the nail was broken beyond fixing. I went to see Dr. Golub, and I couldn’t have asked for a better doctor.

He immediately understood what needed to be done, explained everything clearly, and made me feel calm about the procedure. He ended up removing the toenail quickly and with very little pain. What I really appreciated was how he walked me through the process step by step and gave me clear instructions for aftercare so I could heal properly.

The recovery has gone smoothly thanks to his guidance, and I’m already feeling so much better. It was such a relief after dealing with the pain and worry. Dr. Golub is professional, caring, and skilled — I’d highly recommend him to anyone who needs podiatry care

They were able to squeeze me in as a walk in, I only waited about an hour and had an ingrown toenail surgery same day, I am so thankful for them! They informed me every step of the way, and kept me educated in the process of the surgery as well. The staff is also wonderful, very friendly and makes you feel taken care of. I wish I can put a 5 star review to Dr. Ryan Golub. Unfortunately, it is the opposite, he deserves a 1-star review.

I injured my left foot on April 7th couldn’t walk at all at the next day and call Dr. Golub’s office to schedule an appt. They give me an appt for April 22nd. I arrived in crutches because I had pain every time I stepped on my foot. I put myself in crutches for 2 weeks until I saw him. When I saw him on Apr 22nd, he asked a couple of questions and he diagnosed a muscle sprain in my left foot even though I did not have the purple or exaggerate swelling a common muscle sprain has. Nor did he do an MRI to diagnose, only X-rays. At the end of the appt he prescribed Meloxicam, PT 2x a week for the next 3-4 weeks and he said to remove 1 crutch so I was bearing weight on my injured foot. I saw him again on May 8th 2 weeks later and I came back again with 1 crutch and still limping. When he saw me, he said “again not off of that crutch that’s odd”. He looked at my foot and said do you want an MRI or maybe next time I see you? I said no let’s do it asap, he gave me the order and got an MRI on May 16th. The following week he called me and said MRI results came and you have an issue with your arch and next time I see you we can do a steroid injection and if that doesn’t work then Surgery. I was stunned because of course he didn’t go in details over the phone.

I saw him again on May 28 at this point this was the 3rd appt and there has already been 7 weeks since my injury and I did not have any progress and couldn’t walk properly. I was feeling very disappointed for his poor diagnosis since appt 1 and because I didn’t have any progress at all, I went to see a 2nd opinion and Thanks God I found Dr. Arya foot and Ankle. She LISTENED to my symptoms clearly and she said let’s call radiologist again (MRI) because what you are telling me does not align with the MRI results. She called and had them analyze the MRI Images and they found out a chronic injury in my plantar plate suggested by a cyst and affecting my second metatarsal. This was amended to the MRI report. They found the real root cause due to a doctor listening. Immediately she said stop PT because at this point PT had me do it calf raises, heel rises and exercises a plantar plate injury should not be doing at least not until 4-6 weeks rest which I did not have due to the misdiagnosis of Dr. Ryan Golub. The second Dr. put me in the right treatment going forward and I asked her to forward the MRI results to Dr. Golub’s office.

On the 3rd appt of May 28th I walked in and told him about the MRI amendment and he looked at the amended MRI printed copy and he said oh I didn’t have time to read it let me read it and he saw a cyst and said this is nothing they come and go by themselves.
He did not read the part of chronic injury plantar plate, something very easy to do and he ignored that part.
He assessed my foot and I told him I do not have pain on the navicular bone area (arch) which is what he diagnosed and what the original MRI says. He said the foot is very complex and there could be some damage inside that needs time. At this point I came with a second opinion results and he ignored the amended of the MRI and my symptoms. Now it’s been 3 months since my injury and I am still in rest since May 26 and I am waiting for my plantar plate to heel along with upcoming PT specialized in plantar plate not a common PT. If this doesn’t work there is further treatment for my foot. Due to Dr’s Golub misdiagnosis my treatment is delayed and I have not been 100% in these 3 months. ADVICE: make sure he listens to you and if he is not go and look for a 2nd opinion ASAP do not wait like I did.

From day one Dr. Golub was very professional He reviewed my X-ray and immediately talked to me about the best treatment plan. His medical assistant is super helpful as well. I had additional questions and Art was very helpful. I went back to him and Did my surgery on November 8 and I am happy I did. Definitely recommend him and his staff to anyone looking for a knowledgeable and caring doctor.

Dr Golub was very informative of my situation. He explained everything in detail and made me feel that he really cared. He was delightful and confident. The office is welcoming and I look forward to my next visit with him.

They did excellent work. Always asked how I was feeling and where my pain was in order for them to focus on that area. Also the staff is really friendly. The assistant America was so nice and always asked how I was feeling and even when they were super busy she was always on top of everything and setting up my next appointments before I was done with therapy.

My husband and I started the plan in August. Our coach is amazing. All the coaches are so supportive and encouraging. Revitalize really does tailor guidance specific to your needs. My husband and I have different nutritional needs. I receive different recommendations than my husband on a weekly basis. This program is effective. My husband and I are very happy with our success and the program.

This program was amazing! My coach Cody kept me motivated, educated and on track. We’ve been going to Team 4 Kids for a little over a year now and We can’t say enough great things about my son’s Occupational Therapist, Ms. Taylor. She is amazing and absolutely incredible with my son, who can sometimes be a stubborn cookie! Ms. Taylor is so knowledgeable about her job and always knows exactly how to support and challenge my son in a way that keeps him engaged, motivated, and having fun. Since starting OT, we have seen such an incredible difference, and we are so thankful to have been paired with Ms. Taylor. Our son looks forward to seeing her so much that he even asks to go see her on days when we don’t have therapy, which truly says it all.

My son Michael has been going to T.E.A.M 4 Kids for 2 years already and we’ve seen so much improvement. The therapist are great and the front desk as well. Marixa at the front desk, always makes us feel welcome. She’s always helpful and has great knowledge of what she does.

My boys have only been in speech therapy at the Avondale location for 2 weeks, but man have I seen a difference! They’ve both been in speech therapy since they were 2 years old, but we moved from Indiana to Arizona recently and just started at this location. My older son, Anson (7 yo), has been mostly nonverbal and not very responsive to therapy intervention, but he reached a milestone this last week and is always excited to go! My younger son, Archie (4 yo), is very talkative, but has characteristics of apraxia. He is the first one in the car, when I mention “therapy” or “Miss Madi”, and always protests leaving after his sessions! Ask for Miss Madi, especially if you feel like you’ve hit a plateau in your child’s speech progress!

We’ve been coming here for both Mommy and me group and Speech one on one for a year and a half now with Miss Madi. She’s great with my son; he’s come a long way from when we first started coming! Not only does she make him feel super comfortable with her but also us as a parents. She’s really good at explaining to us how we can better help our child and giving us examples. Very good at hearing any of our concerns. Can’t wait to see how his speech keeps growing in the near future! It’s amazing to hear my son’s voice more and more as time goes by. Everyone has always been very welcoming! Thank you!

I can only speak about the intake at this point but this is the first place that called me after getting the referral from my doctor. I had a couple of places before this that I tried but no one would call me back. TEAM 4 Kids called me first. They happened to call at a bad time but they actually followed up a couple days or so later, which I really appreciate. The intake specialist was very nice and professional. It’s nice to know there are people who know how to do their jobs!

My son had been coming to team 4 kids for about three years now. All therapists have been patient and kind to him. Not only are the therapists supportive and friendly but also the front office team in all locations I’ve been to either Avondale or Surprise office. Morgan from Team 4 Kids Avondale front office has always been super helpful and friendly when I interact with her regarding my son’s appointments. I have not had any negative experience with Team 4 Kids.

My son is having the most wonderful experience at TEAM4Kids!! This is the first time ever we left speech and he wasn’t frustrated or angry. He even noticed that the therapist didn’t make any funny faces at him when he said words wrong. I loved the fact that the therapist talked to him instead of at him and worked down at his level. Every time we drive by he proudly says ‘that is where my appointment is!’ We have already recommended some other families! I tell you this is really the first time he has been happy about speech therapy.

Scottsdale weight loss has been nothing short of amazing! I went in October with a goal in mind due to an upcoming wedding the following April. My goal was 140 but I reached 134 by the time my wedding day came. They are uplifting and encouraging. Helpful tips were always given. I was SO nervous about my first chiropractic visit – total white-coat syndrome! But Dr. Katherine immediately put me at ease from the moment I walked in.

The entire process was seamless: easy online forms before my visit, Niya at the front desk got me checked in smoothly, and Dr. Katherine did a thorough intake, explaining everything in detail.

The MyoVision sEMG test showed exactly what was causing my pain, which was helpful to understand.

They have an amazing table that moves from vertical to horizontal. You are standing and then it slowly lays you down on your stomach – coolest machine ever!

Dr. Katherine used the Activator Method with movement tests to ensure it was working properly. My right leg was short due to postural imbalance and joint dysfunction – my legs were perfectly balanced after the adjustment to my hips and pelvis.

NO cracking, NO twisting – just precise, comfortable treatment that actually felt great. My back felt AMAZING after the appointment!

Dr. Katherine’s gentle approach was comforting and gave me hope that my pain could be resolved for good.

I’ve been to many chiropractors before, so I went in expecting this to be the typical “I give them my full history and then I get twisted and yanked and leave feeling the same and feeling like I wasted money” experience. However, immediately this was completely different. The level of kindness at the front desk, all the way to the sheer knowledge, safety, and care Doctor Dave gave me throughout my hour long intake was *phenomenal*. I felt so heard/seen, especially around explaining these issues I’ve had for 3+ years now that usually just get brushed off, he really listened and explained everything he wanted to do to take care of them. I am fortunate to not have pre-cancer or spots needing to be addressed. All personnel were efficient and pleasant. Highly recommend. In these days of medical mafia taking over practices, it is a pleasure to still deal with an independent office.

The nursing assistant was very nice, personable, and explained everything from beginning to end for my visit.

Teresa was super thorough with my skin concerns and made sure that I was satisfied with the results. She listened to what my concerns were and took care of the concerns I identified and even found more to take care of!

She also removed a concern from my nose and sent it in for a biopsy, I am thankful and grateful. No one has offered that as a solution and I appreciate her care and ability to take care of the issue immediately in office.

I was first greeted by a joyful, helpful and prompt check-in person.
The office was very orderly. The staff were ALL responsive and kind.
The doctor was amazing and treated my skin problems and then gave remedy to areas of concern.
The medical assistant was patient and checked that I had collected my treatment plan and sample of ointment.
The Team attitude was professional, caring and quite efficient. Personal treatment and proper exchange gave way to an extremely good experience. This is a delightful occasion for me and one to receive a 5-Star review!

Friendly, comforting, Professional. Dr is pretty kool with the cryo blaster. The doctor did tricky work around my eyelids. I felt confidence in his efforts. Results were wonderful. And the Doctors assistant was professional and friendly too. Front office was gentle during intake and scheduling. I’d be happy to return.

I had a rapidly growing lesion on my scalp. I called in to set up an appointment. Explained the rapid growth and they gave an appointment for the next day and removed it immediately. Staff was courteous and great.

Receptionists were very friendly and helpful. And the Doctor was very helpful and knowledgeable about the issues I had. They even gave me my phone before I left because I forgot it in the consult room 10/10 experience would recommend.
